Immunizations Alden Montessori School requires all enrolled children provide proof of up-to-date immunizations as mandated by the Texas Department of Health and Human Services, unless an exemption is granted through a signed affidavit as permitted by law.
Immunization Requirements - Parents or legal guardians must submit a complete up-to-date immunization record for their child at the time of enrollment, verified by a licensed healthcare provider.
Parents must provide the school with updated shot record each time the child receives an immunization.
Exemption by Signed Affidavit - If a parent or legal guardian seeks an exemption from immunization requirements for medical, religious, or philosophical reasons, a signed, original affidavit must be submitted to the school office and must remain in the childs file even if child is no longer enrolled.
Compliance - Failure to provide proof of immunizations or a valid signed affidavit by the enrollment deadline may result in delayed admission or exclusion from school until requirements are met.
Statement of Health Parents must obtain a Statement of Health from the childʼs pediatrician stating that the child is healthy and may participate in school activities.

Hearing and Vision In accordance with the Texas Department of Health and Human Services, Alden Montessori School requires all enrolled children to undergo hearing and vision screenings as mandated for their age group. The school facilitates screenings to support early detection of potential issues and promote student health.
Screening Process - Alden Montessori provides hearing and vision screenings for all eligible students in the fall, conducted by qualified personnel from from an outside resource in compliance with Texas Department of State Health Services requirements.
Screenings are required for children aged 4 and older by September 1 of the school year, as well as for students in kindergarten and grades 1, 3, 5, and 7, per Texas law.
Parental Options - Parents may opt out of the school-provided screening by providing written notification to the school office prior to the screening date.
If a child is absent during the scheduled screening or if parents opt out, parents must arrange for a hearing and vision screening to be conducted by a licensed pediatrician or healthcare provider.
