Select a form from the “Schedule services” section.
Currently there is one form de ned for CCC, called “Medication Fitness for Duty Review”.
Some elds are required, for example the Employee name, other elds are already complete, for example the Employer.
Finally, at the bottom of the form, CCC may attach a Job Description. To do this, click “Choose File” or “Browse” depending on your browser. Find the le on the desktop, select it and click “Open”.
Once selected, the le name is shown to the right of the “Browse” or “Choose File” button.
