Team Agreement Sheet
_____ I have also taken the time to read the handbook to my child and explained the expectations, commitment, and responsibilities they have to themselves and their team. If my athlete or I act in conflict with the stated handbook my child’s placement on a Queen City Storm team may be jeopardized.
_____ I understand the following is due on Signing Day: October payment, athlete info sheet, booster sheet & booster payment.
_____ I have read and fully understand my financial commitment to Queen City Storm outlined in this Parent Information Packet and The Booster Handbook
_____ I understand that I am giving my credit card/debit card information, that information may be used if I do not meet payment deadlines to Queen City Storm/Kids First Sports Center.
_____ I understand that my athlete will not be able to practice or compete if account is not kept current.
_____ I am solely responsible for all travel costs associated with out-of-town competitions including transportation, lodging, and food.
_____ If I decide to forfeit my contract I am responsible to pay all Team Fees through the end of the year. The athlete will be ineligible for any classes until fees are paid. Forfeiting must be communicated with the Hospitality Dept and your Lead Coach.
_____ I understand that I will forfeit any monies paid if I choose to leave a team or am asked to leave the program
_____ We understand this is a 7 month commitment from October - April. We are committed to all practices, competitions, award ceremonies, etc.
_____ We have read and understand the guidelines and procedures throughout the Queen City Storm Team Handbook.
_____ I understand that my athlete may not miss practice 1 week, or 2 practices, prior to a competition. An unexcused absence during this time is cause for removal from their team.
_____ I understand that 3 unexcused absences from Oct 15-May 1 is grounds for removal from their team. I understand what constitutes an excused and unexcused absence.
_____ I understand that all competitions are mandatory, and missing a competition is cause for immediate removal from their team. _____ I understand if my athlete’s team receives a bid to US Finals that he/she are committed to attending.
