The vast majority of mapping elements in this report were developed using the Map Room tool and publicly accessible data available through this site; including the American Community Survey, EDfacts, FBI Uniform Crime Report, Feeding America, BRFFS/ACES Related Indicators, DEA Survey Report.
National Health Indicators Warehouse
Provides searchable survey responses for table generation.

Broadstreet
Due to site renovations at Community Commons, content and map building tools were migrated over to Broadstreet’s host site. Both sites were used.

The County Health Rankings & Roadmaps program is a collaboration be- tween the Robert Wood Johnson Foundation and the University of Wiscon- sin Population Health Institute.
