Eligibility
EPIC employees who work a minimum of 30 hours a week are eligible to enroll in our Health Benefit plans. Our comprehensive program provides you with the opportunity to select the best fit for you and your family.
Family Member
Requirements
Your Spouse
Must be your legal spouse for federal
tax purposes including state recognized common-law spouses and same gender spouses. Ex-spouses are NOT eligible for active coverage, even if court ordered.
Your Domestic Partner
Must be a company-registered domestic partner*
Your Children, Including: Biological and adopted children
Disabled children who depend on you for support
Children you are required to cover under the terms of a Qualified Medical Child Support Order (QMCSO)
Must be:
Under age 26
A disabled child of any age
Your Benefits Online
You can manage your benefits elections on our employee Benefits Portal. From annual enrollment to Qualified Life Event Change—It’s all there! First-time users will need to register for a user account. Click here for a step-by-step guide.
If you are ready to elect your benefits visit our employee Benefits Portal. Remember to verify your personal information, make any necessary changes then make your benefits elections. You will not be able to change your elections until the next Open Enrollment period unless you experience a Qualified Life Status Change.
* In order to include a Domestic Partner on our benefits Plans, you must submit an Affidavit of Domestic Partnership and a Declaration of Tax Status. If your domestic partner does not meet the IRS definition of an eligible dependent, additional contributions for his/her coverage will be taken on an after-tax basis.
