1. Visit www.Teladoc.com or call 1-800-Teladoc.
2. Click Set up account.
3. Provide the required information.
4. Consultation cost of:
EPO Plan $25 PCP copay. POS Plan $20 copay.
By completing your medical history disclosure (MHD) ahead of time, Teladoc doctors can assist you more ef ciently and make an accurate diagnosis.
