Benefits
Overview
IPSoft, Inc. is proud to offer a comprehensive bene ts package to eligible, full-time employees working 30+ hours per week. The complete bene ts package is summarized brie y in this booklet. For more detailed information about each of these programs, please visit http://url2come.com
You are eligible for employer-sponsored and voluntary bene ts. You share the cost of some bene ts – Medical, Dental, STD, LTD. IPSoft, Inc. provides other bene ts at no cost to you – Basic Life and AD&D. In addition, there are voluntary bene ts
– Vision, Voluntary Life and AD&D, with reasonable group rates that you can purchase through IPsoft, Inc. payroll deductions.
Open Enrollment & Benefit Changes
Employees have the opportunity to enroll or make changes to their bene ts during these times:
Open Enrollment — OR —
Qualifying Life Event (QUE)
This includes marriage, birth of a child, divorce, legal separation, employment status, dependent eligibility, death, loss of coverage.
* You must notify Human Resources within 30 days of a qualifying event. (60 days to notify of birth of child).

⸋ Your Children, Including: Biological and adopted children
⸋ Disabled children who depend on you for support
⸋ Children you are required to cover under the terms of a Quali ed Medical Child Support Order (QMCSO)
Eligibility
⸋ You and your dependents are eligible for IPsoft, Inc. bene ts if you are full-time emloyees working 30+ hours per week.
⸋ You are eligible for Medical, Dental and Vision coverage the 1st of the month following your date of hire.
⸋ You are eligible for Life and AD&D, STD, LTD and FSA on the 1st of the month after 3 calendar months of service.
Loss of Dependent Eligibility: It is your responsibility to notify Human Resources of any dependents whom are no longer eligible for our plan bene ts. This includes ex-spouses or dependent children who have aged off the plan, are married or lose their dependent tax status. If you fail to remove a dependent when he or she becomes ineligible, you will be required to pay the insurance premium and risk the insurance company denying the claim.
2018 Benefits
