Basic Life and Accidental Death & Dismemberment
To ensure that all eligible associates have some level of financial protection, Zelis provides employer paid Basic Term Life and AD&D insurance to full time associates. Both policies are provided at no cost to you. You are automatically enrolled in Basic Term Life and AD&D Insurance when you become eligible for coverage.
Benefit Amount
Associate: $75,000 / Dependent: $5,000 (spouse & each child)

AD&D
Your Basic Life coverage includes Enhanced Accidental Death and Dismemberment coverage. The AD&D amount matched the Basic life benefit amount.
Short Term Disability
When you need to miss work for an extended time due to a non- occupational illness or accident, short-term disability insurance can replace a percentage of your lost income for a certain number of weeks (to a maximum of 26 weeks). This benefit is 100% paid by Zelis and administered through Guardian.
Weekly Benefit: 80% for the first 13 weeks;
60% for the following 13 weeks; up to $2,000
Elimination Period: 7 days Maximum Benefit Period: 26 weeks
This benefit is available to all associates including those associates who are eligible for state mandated disability insurance such as New York, New Jersey, California and others. Please note: Your state mandated short term disability benefit amount will offset your short term disability amount.
Long Term Disability
Long Term Disability is 100% paid by Zelis. LTD plans provide you with financial protection if you have to miss work due to a non-occupational disability. Benefits from a disability plan will supplement your income to help you pay for your day to day household expenses, as well as copayments and other medical costs not covered under other plans.
Monthly Benefit: 60% of monthly earnings: up to $10,000 Elimination Period: 180 days
Maximum Benefit Period: Social Security Retirement Age Disability: 2 years own occupation / any occupation thereafter
