You have the choice of multiple medical plans:

FLEXIBLE SPENDING ACCOUNT (FSA)
Health Care Flexible Spending Account (FSAs) help you pay for qualified healthcare expenses with tax free dollars.
Limited Purpose Flexible Spending Account (if enrolling in the Anthem HSA PPO) help you pay for qualified dental and vision expenses with tax free dollars.
PREVENTATIVE CARE COVERED AT 100%
Getting regular checkups and exams can help you stay well, catch problems early and may even save your life. Your medical plans offer preventative services at no cost to you.
DENTAL PLANS
As a benefit-eligible employee, you may choose between the following dental plans; Delta Dental HMO,** Delta Dental PPO,* and Kaiser Dental (Hawaii only).
VISION PLAN
In addition to network VSP providers, VSP members will have access to participating retail chains like Costco optical, Shopko, Visionworks, Cohen’s, Optyk, & Heartland Vision.
LIFE AND DISABILITY PLANS
Skechers offers an array of Life and Disability plans that are employer paid and employee paid. When used in the right combination, it can protect your income should the unexpected happen.

ELIGIBILITY FOR U.S. DOMESTIC AND PUERTO RICO (EXCEPT HAWAII AND CIGNA INTERNATIONAL)
You may become eligible for benefits under our plans in one of the following ways:
1. You can qualify as a full-time employee eligible for benefits when originally hired to regularly work 40 or more hours a week (coverage effective on the 31st day of employment);
2. You may qualify for benefits if you are a part-time employee and have averaged working 30 hours or more per week during a 12-month measurement period as defined by Skechers.
Part-time associates meeting the average 30 hour per week requirement will be notified of how to enroll when they become eligible.

YOUR DEPENDENTS
You may also enroll your eligible dependents in the Skechers Benefit Program. You will need to provide document substantiation for each dependent you enroll. Refer to the “Enrollment” section of this guide for a list of documents we will accept. Your eligible dependents include:
• Legal spouse
• Qualified domestic partner*
• Dependent children up to age 26 (including stepchildren, adopted children, and your domestic partner’s children*)
• Disabled children who have reached the maximum age and who are (or become) physically or mentally incapable of self- support (medical certification required)
*The IRS considers coverage for domestic partners a taxable benefit that must be included in your gross income.
YOUR PAYCHECK COSTS
When you become eligible for the Plan, your benefits become effective on the 31st day of employment and you will be responsible for all per paycheck costs in the pay period in which your 31st day falls. If you become ineligible for the plan, your benefits terminate on the last day of
the month in which you become ineligible, and you will be responsible for all per paycheck costs for the month. Benefits costs are NOT prorated.

Scheduled to work 40+ hours per week.
PART-TIME
Working 30 hours per week on average over a 12 month period.
