SOP Title: Rejection of Performance Site Review Request in Velos
• QS requests (e.g. completion of a PSR application, Delegation of Authority (DOA), 1572, Service Order Agreement (SOA), Service Package Grant to cover RA coordinator time) to members of research team have not been completed.
5. SPECIFIC PROCEDURE
• Members of Quality Specialist team will identify studies where a PSR request has been in their queue for ≥ 90 days.
• Members of the QS team will document the list of concerns and dates where e - mail requests were sent to the research team. A report of concerns and timeline of actions will be created for each identified project that is eligible for PSR rejection.
• Members of the QS team will schedule time with the RA Educator and Director of RA to review the project. A decision to reject the PSR will be made following the meeting.
• For those projects that meet PSR rejection criteria and have received internal approval from CHRA management, QS will follow-up with a letter to the research team which explains the decision behind rejection of their PSR and provide guidance on corrective actions and opportunity to resubmit their PSR application for a follow-up review.
