In Section III, you will need to provide your institutional access user ID. This is the user ID assigned to you by your hiring institution.
In this section, list every User ID you are associated with at UT Southwestern, Children’s and Parkland.
If you do not have a User ID for any of the Institutions listed, enter “NA”.
Remember, if you are submitting on behalf of someone else, the requested information is in reference to their User ID.
Enter their User ID and not your own.
