SOP Title: Rejection of Performance Site Review Request in Velos
• QS questions have not been addressed by the PI or members of the research team.
• QS requests (e.g. completion of a PSR application, Delegation of Authority (DOA), 1572, Service Order Agreement (SOA), Service Package Grant to cover RA coordinator time, etc.) to members of research team have not been completed.
5. SPECIFIC PROCEDURE
• Members of Quality Specialist team will identify studies where a PSR request has been in their queue for ≥ 90 days.
• Members of the QS team will document the list of concerns and dates where e- mail requests were sent to the research team in their study folder.
• For those projects that meet PSR disapproval criteria, QS will follow-up with a letter to the research team which explains the decision behind disapproval of their PSR, and QS will provide guidance on corrective actions and opportunity to resubmit their PSR application for a follow-up review.
