On-Site Research Monitor Requirements The following instructions must be followed for all on-site monitor visits. If you

All monitors coming to any CMC campus must
• Be screened by the concierge upon entering the facility. The concierge will process their driver license through the FastPass system and give them a visitor badge. There is no FastPass system at the Specialty Center. All monitors will need to be taken to the main hospital to be checked in and receive a visitor badge.
• Be escorted by research staff at all times.
In addition, if they need to visit any clinical/patient care area(s), they must also
• Register with Vendormate prior to their visit.
• Be compliant with all documentation (policies & vaccination)
requirements.
• Check-in at the Vendormate kiosk and receive clearance.
**Effective 10/1/2021, all on-site visitors must be fully vaccinated against Covid 19. More detail will be provided as it becomes available.

The annual credentialing fee covers you, your company, and all its representatives who interact with us. The amount varies depending on your company's profile and is paid directly to our partner, Vendormate, Inc.
