How to Request Epic Access for Monitors:
1. Please fill out the Request for Access to Medical Records for Research Purposes form (page 2) and email it to HIMResearchRequests@childrens.com
a. Pleasecontactyourmonitorandobtainapreferred4-digitpersonal identification number (PIN). This is a 4-digit number of their choosing that will need to be added to the form before access can be ordered.
2. Study Consents can be emailed to HIM.Transport.FAX.documents@childrens.com to be uploaded to Media Manager on the patient
**If the monitor needs to change their access dates or add a few more patients, please let the HIM Department know
How to View Medical Records:
1. Go to www.childrens.com/epic or click here and follow the steps below:
2. Call the IT Service Desk 214-456-8999 a. pressoption#1forpasswords
3. When the agent comes on the line you will need to provide the following:
a. YourName,
b. Company Name, c. ContactNumber d. 4 digit PIN
4. Refer to the Ticket Number listed below in yellow
5. You will be provided with a password for your ECLMONxx account
6. Please test the password while the service desk agent is on the line
