iv) Be sure to upload a red-lined (track changes on) copy to section 4.3.
v) Click the consent form link in question 4.4. This will take you to the parent protocol smartform. In the parent protocol smartform, upload the clean (track changes off) version of the revised consent form to section 54.1.1a. If a consent form was loaded initially, then you will click the upload revision link instead of add.
vi) Click save, then exit to get out of the parent protocol smartform and return to the modification form.
