Using Figure 4.1 as a suggested format, design a questionnaire that is appropriate to your class. You can design an open-ended ques- tionnaire in which the learners are free to comment on any learning/ teaching item(s) they wish, or a closed questionnaire in which you es- tablish specific items on which they must focus. If you wish to design an open-ended questionnaire, ask the learners to insert in the blanks any item or items on which they wish to give feedback. If you wish to design a closed questionnaire, insert in the blanks the specific learning/ teaching focus. It can be a particular activity within a lesson, a vocab- ulary item, a grammatical item, a task, an exercise, or a combination of any of these. Feel free to reformulate the statements to make them suitable to the proficiency level of your students.
