UNPAID HOLIDAYS
                 Time  off  will  be  provided  to  employees  who  are  veterans  on  Veterans  Day,
                 November  11,  if  the  employee  would  normally  be  required  to  work  that  day.
                 Employees  wishing  to  take  the  day  of  must  provide  their  supervisor  and  Human
                 Resources with at least one month’s prior written notice of his or her intent to take
                 time off for Veteran’s Day and must also provide a federal certificate of release or
                 discharge from active duty, or similar federal document, for purposes of determining
                 the employee’s eligibility for the benefit.

                 BEREAVEMENT LEAVE
                 When  a  death  occurs  in  an  employee’s  immediate  family,  all  regular  full-time
                 employees may take up to three (3) days off with pay to attend the funeral, make
                 funeral  arrangements,  or  handle  matters  related  to  a  death  in  their  family.
                 Bereavement  hours  provide  paid  time  off  in  lieu  of  scheduled  work  hours.    The
                 Company may require verification of the need for leave.  Immediate family members
                 are defined as a spouse, child, father, mother, brother, brother-in-law, sister, sister-in
                 -law,  father-in-law,  mother-in-law,  grandparents,  and  grandchildren.  You  will  be
                 given  1  day  unpaid  time  off  for your  spouse’s  grandparents.   Additional  time  off
                 may be requested and granted without pay or taken as PTO leave.  Employees shall
                 be paid for the hours they were scheduled to work based on a 12, 10, or 8 hour day.

                 ILLINOIS: CHILD EXTENDED BEREAVEMENT ACT
                 A full-time employee who has been employed for at least two (2) weeks is entitled to
                 use a maximum of six (6) weeks of unpaid leave if the employee experiences the loss
                 of a child by suicide or homicide.

                 "Child"  includes  the  employee's  biological,  adopted,  or  foster  child,  a  stepchild,  a
                 legal ward, or a child of a person standing in loco parentis.

                 Child extended bereavement leave  may be  taken in a single  continuous period or
                 intermittently  in  increments  of  no  less  than  four  (4)  hours,  but  leave  must  be
                 completed within one (1) year after the employee notifies the Company of the loss.
                 The Company may require reasonable advance notice of the employee's intention to
                 take  leave,  unless  providing  such  notice  is  not  reasonable  and  practicable.  The
                 Company also may require reasonable documentation. Documentation may include
                 a death certificate, a published obituary, or written verification of death, burial, or
                 memorial  services  from  a  mortuary,  funeral  home,  burial  society,  crematorium,
                 religious  institution,  or  government  agency.  The  Company  may  require  that  the
                 documentation include the cause of death.
