Admission into the Communications Lineworker Program (CLP) begins by individuals meeting  ve basic requirements for enrollment:
• Must be at least 18 years of age by the beginning date of enrollment.
• Must provide o cial high school transcript or GED.
• Must pass a Department of Transportation (DOT)
physical examination and have the physician sign
the SLTC Doctor Release Form.
• Must posses a valid driver’s license from state in
which you reside.
All students enrolled in the CLP must maintain medical coverage during the entire term for which they are enrolled. Proof of coverage is not required with the application, but will be required in the form of a valid certi cate of medical coverage prior to the start of the term for which the student is applying.
If you do not currently have medical coverage, contact a medical coverage provider and ask about short-term medical coverage programs. Blue Cross Blue Shield of Georgia has provided short-term coverage to many CLP students. If you would like additional information, please contact a school representative or visit

1. Read and sign “Enrollment Agreement”.
2. Sign the “Release and Waiver of Liability and
Indemnity Agreement”.
3. Sign the “Medical Coverage Release Form”.
4. Visit your local physician and have them
complete and sign the “Doctor Release Form”.
5. Enclose a copy of Medical Examination Report
(DOT physical).
6. Have your school provide an o cial copy of
your transcripts or your GED certi cate.
7. Enclose a photocopy of your current valid
driver’s license.
8. Enclose payment of $750 for the prepaid lab and
application fees. Make check payable to SLTC.
9. Upon submi ing the above items, please allow up to four weeks for noti cation of acceptance
into the program. An SLTC representative will contact you by phone and will also mail a le er of acceptance.
