P. 4

1.
Pre-planning
Careful planning is needed to prepare for
the NDIS. It is important to think about what your life is like now, what your immediate support needs are and what your current and future goals might be. It is also important to gather information from your existing support providers, allied health professionals, specialist doctor or GP to understand your support needs
2.
Developing your plan
You will be contacted by the National
Disability Insurance Agency (NDIA) to set up a planning meeting. To create your first plan you will meet with an NDIS representative or Local Area Coordinator to have a conversation about your current situation and supports. Your plan may include informal supports, community supports, mainstream supports, and reasonable and necessary funded supports. A representative can be there to help.
3.
Starting your plan
Once your plan is approved, it is time to
action it. There are supports available to help you implement and monitor your plan, this is called coordination. Support Coordinators will link you with providers such as Just Better Care, address any barriers to accessing services and help to gain more independence.
4.
Turning your plan into action
Just Better Care believes each person
has the right to choice and control over their life, so each local office has a range of options to support you in all aspects of your life. We take the time to get to know you, identifying the best ways to support you in achieving your goals - whether it’s career aspirations, exploring a passion or travelling.
