Steps to Prepare for the NDIS Planning Process
This booklet is a guide to assist you in considering how you currently live your life and how you would like this to look in the future. The booklet reviews your current living arrangements, service needs, medical needs, important people in your life, social and emotional connections, and what is important to you moving forward.
 Goals or Vision
Just Better Care will assist you in completing the booklet and guide you through the questions; however, you do not have to complete the booklet if you do not wish to.
It may be helpful to discuss these questions with the important people in your life e.g. family, health professionals, teachers or employers:
• What supports and services do you need to do everyday activities?
• How would you like your life to be in a few years?
• How can the NDIS help you?
When considering how NDIS can help, you may wish to provide documents evidencing:

The NDIS Planner will consider the supports that will help you achieve your goals, increase independence and enable participation in the community.
The NDIS can assist in funding reasonable and necessary supports, which may include assistance with employment, therapies, home modifications, mobility equipment or vehicle modifications.
