Body Map is to be used by carers to record any injuries to service user and to explain their presence
Please draw on the body map in black ink, using the following key to indicate the different types of injury (shading or alphabetic code), and provide brief details for each injury,
e.g. measurements of wound, colour of bruise, etc using arrows
(a ruler is provided to assist with measurement)
A- pressure ulcers C- cuts & wounds E- scalds & burns
B- bruising D-excoriation, red areas F- other (specify) (not broken down)
