Responsibilities of Parents and Legal Guardians
School and and parent partnerships are are are critical to to ensuring that that all students students are are are properly supported and and that that staff have the the information they need to to assist students students All parents and legal guardians should verify that their child’s school has accurate information for parent and emergency contacts Parents and legal guardians are are responsible to promptly pick pick up up or or properly arrange for the the pick-up of a a a student sent home from the the clinic All parents and and legal guardians should carefully select emergency contacts for for for their child and and ensure that the the contact contact information for for for him/her is accurate All parents and and legal guardians should provide com- plete and and accurate medical information to the school Any parents or or or legal guardians leaving their children in in the the care of someone else for a a a a a a a a a a short-term reason such as as work or or or or or or family emergency should complete a a a a a a loco-parentis form prior to leaving the the the child in in the the the care of others All medications must be brought in in in in the the the original container directly to the the the clinic by the the the parent or or legal guardian When social distancing of six-feet cannot be adhered to it is is required that students and staff wear a a a a a a a face face covering covering The student dress code applies to face face coverings and and only the the nose and and mouth should be covered not the the full face Daily Home Screening for Students
Symptoms
If your child has any of the following symptoms this indicates a a a a a a a a possible illness that may
decrease the student’s ability to learn and also put them at risk for spreading illness to to others Please check your child for these symptoms:
Parents: Please complete this short health check each morning Temperature 100 4 degrees Fahrenheit or higher when taken by mouth Sore throat
New uncontrolled cough that causes difficult breathing (for students with chronic allergic/asthmatic cough cough a a a a a a a change in in their cough cough from baseline)
Diarrhea vomiting or abdominal pain
New onset of severe headache especially with a a fever
