When home health is ordered, a social worker may do an assessment and ask questions for the patient and family. Social workers address a number of issues affecting patients including end-of-life decisions, homelessness, facility resources and medication adherence. Social workers focus on strengthening the patient’s network of support to address the patient’s need. In some cases, social workers may
be called upon to do a psychosocial assessment and establish an intervention plan if necessary. Working with a physician, a medical social worker can provide counsel and find appropriate resources for the patient.
