During maintenance work in the primary crusher, an incident occurred, and a welder was hit by a rock with a diameter of about 10 cm. Although a prior clean-up had been carried out to remove the rocks, the rain and the lack of lighting created by a power outage contributed to this incident. In order to prevent this from happening again, and as a temporary measure, a rented mechanical shovel was made mandatory at each entry into the crusher, in
order to manage the risk of rockfall. However, this could not be the permanent solution.
After investigation and analysis of the incident, and based on feedback from workers and supervisors, a system of containment for the accumulated rocks at the exit of the rock box was determined to be the best long-term solution. With the support of the engineering and health and safety
departments, a custom-built guillotine
system that takes into account various
structural elements was developed and
implemented. In addition to making the work
safer, the guillotine system is easy to use for
workers. It is attached to the structure in
place and a hydraulic system allows it to go
down and block the exit. With the guillotine in
the lower and closed position, the risk of
falling stones is now virtually
  non-existent.
