PARTICIPATE in all decisions about your treatment. Being aware of your care is the best way to prevent errors.
EDUCATE YOURSELF
about your illness. Understand your diagnosis, treatments and tests.
Take notes and write down any questions you may have.
ASK a trusted family member or friend to be your advocate. They can help you remember questions you want to ask.
KNOW your medicines and why you take them. Keep a list of your meds with you, also any allergies or reactions you may have.
