When You Get Home
• TELL your healthcare team before you leave the hospital, if you need home health services for help with bathing, shopping, cooking or taking your medicines.
• DON'T RUSH! Trying to do everything you used to do before going into hospital could delay your recovery.
• REST when you need to. For the  rst few days
you may tire quickly–don't overdo it!
• ASK a family member or friend to CHECK ON YOU AS YOU RECOVER.
• PLACE your phone and emergency numbers within EASY REACH.
• CONTROL YOUR PAIN! This will help to reduce stress and help you to get the rest you need.
• TAKE MEDICINES AS DIRECTED by your doctor and manage other chronic conditions like diabetes, heart disease or COPD.
• USE THE MEDICINE MINDER, APPOINTMENT KEEPER and NOTES pages (25, 26 and 37) to keep track of your recovery.
• TELL YOUR DOCTOR
how you feel throughout your recovery and ask questions about your care plan or your progress.
