Your Patient Responsibilities
1. Provision of Information
A patient has the responsibility to provide, as possible, accurate and complete information about present complaints,
past illnesses, hospitalizations, medications, and other matters relating to health. The patient has the responsibility to report unexpected changes in health condition to the responsible practitioner. A patient is responsible for reporting
clear understanding of a contemplated course of action and what is expected.
2. Compliance
A patient is responsible for following the treatment plan recommended by practitioners primarily responsible for care. This may include following
the instructions of nurses and allied health personnel as they carry out the coordinated
plan of care and implement the responsible practitioner’s orders, and as they enforce the applicable facility rules and regulations. The patient
is responsible for keeping appointments and, when they are unable to do so for any reason, for notifying the responsible practitioner.
3. Participation In Pain Management
The patient is responsible for reporting pain to healthcare personnel and for informing personnel of the e ectiveness of treatment provided.
4. Refusal of Treatment
The patient is responsible for their own actions if treatment is refused or the practitioner’s instructions are not followed.
5. Facility Charges
The patient is responsible for assuring that the  nancial obligations are ful lled as promptly as possible and for knowing the terms of their insurance policy.
6. Concerns/Complaints
The patient is responsible for sharing concerns or complaints to the care giver or to the appropriate manager. Such concerns or complaints may be made verbally or in writing.
7. Respect and Consideration
The patient is responsible for being considerate of the rights of other patients and facility personnel and for assisting in the control of noise and the number of visitors.
The patient is responsible for being respectful of the property of other persons and of the facility.
Privacy and Health Information
Your health information is protected by the Privacy Rule, a federal law that gives you rights over your health information and sets rules and limits on who can look at and receive your health information. When you are admitted, you will receive a copy of our Notice of Privacy Practices.
Who must follow this law?
• Most doctors, clinics, hospitals, psychologists, chiropractors, nursing homes, pharmacies and dentists
• Health insurance companies, HMOs, company health plans, Medicare and Medicaid
What information is protected?
• Information your doctors, nurses, and other healthcare providers enter in your medical record
• Conversations your doctor has about your care or treatment
• Information about you in your health insurer’s computer system
• Billing information
• Your Mental Health professional’s private
notes about your counseling sessions
What rights do you have over your health information?
You have a right to:
• Ask for a copy of your health records • Make corrections to your records
• Receive a notice that tells you how your health information may be used and shared
• Decide if you want to give your permission before your health information can be used or shared
• Get a report on when and why your health information was shared
If you believe your rights are being denied or your health information isn’t being protected, you can  le a complaint with your provider or health insurer or with the U.S. Government.
Who CAN see and receive your health information?
Your information CAN be used and shared:
• For your treatment and care • To pay doctors and hospitals
for your healthcare
• With your family, relatives, friends, or
others you identify who are involved with your healthcare or your healthcare bills, unless you object
• To make sure doctors give good care and nursing homes are clean and safe
• To protect the public health, such as by reporting when the  u is in your area
• To make required reports to the police,
such as reporting gunshot wounds
Without your written approval, your health information CANNOT: • Be given to your employer
• Used, sold or shared for
marketing or advertising purposes
