Student Handbook and Student Code of Conduct Annual Acknowledgement Form 2022-23
Please review the Plum Borough School District Student Handbook and Student Code of Conduct with your child(ren). These documents are found under the Resources tab of the District’s website www.pbsd.net and are to be reviewed annually with your child.
We ask that all parents and guardians log on to the Sapphire Community Portal and fill out the annual Student Handbook and Code of
Conduct Acknowledgement form for each
student who attends the Plum Borough School
District.
You can access this form via this Sapphire link. Once you are logged in, click on your child’s name. You can then select Student Data Forms on the left-hand side. You will then fill out all forms under the Forms Awaiting Completion title. You can view the Parent Community Web Portal guide for additional details.
This form will be kept on file to acknowledge that you have reviewed the Handbook and Student Code of Conduct. Please fill out this verification form by September 9, 2022.

If you did not pre-register to the Parent Community Web Portal. From there, select Create a Sapphire Community Portal account.
