EMPLOYEE HEALTH CLINIC
If you are injured on the job, you must go to our care provider (at one of the locations listed below) in order for the Township to pay for your medical expenses. The only exception is true life threatening emergencies which must be handled by emergency service professionals and a 911 call. If you go to your family doctor, the Township is not responsible for your medical bills.
You must also ask a supervisor or administrator for permission to seek treatment. There is an authorization form that is available from the Recreation office that you should bring with you when you seek treatment. Phone authorization can be made when you cannot stop for a form prior to receiving treatment. You may get authorization by calling the office during regular business hours – after hours you must always contact the Director or Assistant Director.
