Dear Mr. Hoover:
Your Medicare evaluation is enclosed. Please check the information on the client data sheet on
page 5 to make sure it is correct.
As I mentioned in an earlier e-mail, you and your wife will need to enroll in Part A and Part B
effective January 1. You’ve probably already done this, but if you haven’t you can enroll by
calling Social Security at 800-772-1213. Within two or three weeks after you call, you should
receive your Medicare card in the mail indicating that your Part A and Part B coverage will begin
on January 1, 2019.
Also, if you enroll in one of the plans in your evaluation, here are the steps to follow:
1) To enroll in Medicare Advantage plan, call the plan’s toll-free number that’s shown
on pages 2-3 of the evaluation. Be sure to specify the exact name of the plan – there
are several Humana plans, for instance, and it’s important to make sure you are
enrolling in the one you want.
2) If you decide to get a Medigap policy, you will need to take two steps:
a) choose the plan you want (F, G, HDF, etc.) and then buy your policy directly
from the insurance company that you choose. We recommend that before
buying you call at least three insurers to get current quotes. The toll-free
numbers of the insurance companies licensed to sell Medigap policies in
Indiana are shown in Appendix B1;
b) enroll in a Part D stand-alone plan by calling the plan’s toll-free number
shown on page 4 of your evaluation.
All your physicians accept Medicare-approved rates
As shown in Appendix A, the Medicare web site indicates that all your doctors accept Medicare-
approved rates. When a doctor does not accept Medicare-approved rates, he or she can charge
