               Dear Mr. Stiepel:

Your Medicare evaluation is enclosed. Please check the information on the client data sheet on
page 5 to make sure it is correct.

Enrolling in Part A and Part B

As I mentioned in an earlier e-mail, you can enroll in Part A and Part B in January, although your
coverage will not begin until April 1. Probably the easiest way to enroll is to call Social Security
at (800) 772-1213 and let them know you want to enroll in Parts A&B effective April 1. You
should receive your Medicare card in the mail within two or three weeks after you call.

Until you begin receiving your monthly Social Security payments, you will be billed quarterly in
advance for your Part B premiums. Later, when you start receiving Social Security payments,
your Part B premiums will be deducted from your monthly checks. In your evaluation, I’ve
assumed you will pay the standard Part B premium of $148.50 a month. People with higher
incomes may pay more – the amounts are indicated in the attachment to this letter.

Enrolling in your supplemental coverage options

In your questionnaire, you indicated that you want to enroll in a Medigap plan, which covers
you with all your health care providers. Because Medigap policies do not cover prescription
drugs, you will also need to enroll in a Medicare prescription drug plan, usually referred to as a
stand-alone drug plan. Here are the enrollment steps for each type of coverage.

 First, decide which Medigap plan you want. That will likely be Plan G, the most
comprehensive Medigap plan available to you, although Plan N also has strong
coverage. Then it is a good idea to call at least three insurers for current quotes
before getting your policy. The phone numbers of the insurers licensed to sell
Medigap policies in California are in Appendix B1. Specify that you want your
policy to go into effect on April 1.

 To enroll in the stand-alone drug plan that has the lowest costs for your Rx
drugs, call the plan’s toll-free number, shown on page 4 of the evaluation and in
