               Dear Mr. Newman:

Your Medicare evaluation is enclosed. Please check the information on the client data sheet on
page 5 to make sure it is correct. Because you want your coverage to begin June 1, 2018, you
should purchase your Medigap policy and enroll in a Part D stand-alone plan within the next
week or so. That way, you will have your enrollment cards by June 1.

It is likely that when you enroll in your Part D stand-alone plan and (less likely) that when you
purchase a Medigap policy, you will be asked to verify that your COBRA policy is considered
“creditable coverage.” Medigap policies sometimes have up to a six-month waiting period
before they cover pre-existing conditions, but that period is waived if you have had creditable
medical coverage for the previous six months.

Most of the costs shown in the evaluation are for the entire 2018 plan year, even though you
will be enrolled only for the last seven months of the year.

Enrolling in the coverage you choose

Medigap policy – You’ll get your Medigap policy from an insurance company (or
insurance agent). Each company’s phone number is listed in Appendix B3. We suggest you get
at least three quotes before deciding which company you will get your policy from. And when
you get your policy from the company you’ve chosen, specify that you want your coverage to
               start on June 1.

Medicare Part D stand-alone plan – This prescription drug plan provides your Rx drug
coverage. You can call the plan to enroll – phone numbers are shown on page 4 of your
evaluation – or you can call 800-MEDICARE. Again, let the person you speak with know that you
want your coverage to begin on June 1.

               Medigap policy choices

Earlier you’d indicated that you want the best coverage. Since you do not have an employer-
sponsored retiree health plan, the best coverage will be Medigap Plan F. All told, there are 10
Medigap plans, each with a slightly different set of benefits, as shown on pages 6-7 of your
evaluation. Also, pages 2-3 show more detailed side-by-side comparisons of four of these plans
