               Dear Mr. Levy:

               Your Medicare evaluation is enclosed. Please check the information on the client data sheet on
               page 5 to make sure it is correct. It’s a good idea to enroll in the supplemental coverage that
               you choose by mid-February so that you will have your enrollment cards before March 1.

               Also, within the next couple of weeks you should receive your Medicare card indicating that
               your Part A and Part B coverage is effective 3-1-2018 (if you haven’t already done so, you can
               call 800-772-1213 to enroll in Part A and Part B).

               After you’ve decided on the coverage you want, you can enroll in one of the following ways:

                            If you decide to enroll in an Advantage plan, you can either call 800-MEDICARE
                              to enroll or you can call the plan’s toll-free number, shown on pages 3-4 of your
                              evaluation. The Advantage plans in your evaluation include Rx drug coverage.

                            If you decide to get a Medigap policy and a Part D stand-alone plan, you can
                              enroll in the Part D plan by calling 800-MEDICARE or by calling the plan’s toll-free
                              enrollment number, shown on page 4 of your evaluation. As for Medigap
                              policies, it’s a good idea to call a minimum of three insurance companies for
                              quotes before deciding which company you will buy from. Companies’ phone
                              numbers are shown in Appendix B3.

               Since you are more than six months past your 65  birthday, when you enroll in a plan you will

               probably be asked to show verification that you have had creditable health and Rx drug
               coverage from your wife’s employer plan. Also, you may need new prescriptions from your
               doctor(s) in whatever plan you choose. But first ask if your current prescriptions can be
               transferred to your new plan.

               Health Savings Account

               On your questionnaire you indicated that you are contributing to a Health Savings Account. As
               you may know, after you are enrolled in either Part A or Part B, you can no longer contribute to
               an HSA.
