               Dear Mr. Johnson:

               Your Medicare evaluation is enclosed. Please check the information on the client data sheet on
               page 5 to make sure it is correct. The costs shown in your evaluation are estimates for the 2019
               plan year, although you will be enrolled in Medicare for only nine months of the year.

               You can enroll in Part A & B any time after January 1 by calling Social Security at (800) 772-1213
               (or you can enroll online at ssa.gov or by scheduling an appointment at your local Social
               Security office). Within 2-3 weeks after you enroll, you will receive your Medicare card in the
               mail indicating that your Part A & B coverage will start on April 1.

               In signing up for your supplemental coverage, here are the steps you might follow.

                            Review the evaluation and determine whether you want 1) to get a Medigap
                              policy and a Part D stand-alone plan; or 2) to enroll in the Medicare Advantage
                              plan that’s compared in your evaluation.

                            If you decide on the Advantage plan, you can enroll by calling the plan’s toll-free
                              number shown on pages 2-3 of the evaluation. Make sure that the person you
                              speak with understands that you want your coverage to begin on April 1.

                            If you decide to get a Medigap policy and a Part D stand-alone plan, you should
                              first decide which Medigap plan you want – Plan F, Plan G, Plan N, or another
                              plan. Then we suggest that you call three or more insurers before deciding which
                              company you’ll get your policy from.

                              The toll-free numbers of insurance companies licensed to sell Medigap policies in
                              Texas are shown in Appendix B4, and their estimated premiums are shown in
                              Appendices B1 through B3. Once you’ve decided on the company you will get
                              your policy from, you can enroll on the phone – again specifying that you want
                              your coverage to begin on April 1.

                            Finally, you can enroll in the Part D stand-alone plan that you choose by calling
                              the plan at the toll-free number shown on page 4 of the evaluation (and also in
                              Appendix D2).
