9.3 Crane Workshop
The crane workshop consists of large work vans used by the technicians to move persons and tools around the terminal from crane to crane.
9.4 Service Vehicle Workshop
a) The service vehicle maintenance area was shown to Trent as being on a paved area, opposite the large stores, next to a container, where it is assumed some technicians work from.
b) Certainly not an rea conducive to effective and proper maintenance.
9.5 Lock Out Tag Out (LOTO)
a) KFTL does not appear to have a Lock Out – Tag Out (LOTO) system to safely manage the taking of equipment out of service for PM, BD or any other outage.
b) This is a critical process, and is an absolute necessity, to ensure safe work practices are used at all times.
c) It is Trent’s recommendation, that KFTL engage the necessary resources, either internal or external to develop and implement a sound and workable LOTO system.
