Prescription Drug Coverage – Mandatory
Generic Program
If you decide to buy a brand name medication when a generic is available and appropriate, you will pay a higher copay. Please review the United formulary and discuss other lower cost medication options with your doctor. For more information on lower cost options, log

Prescription Drug Coverage – Rx Step Therapy
Step therapy requires the use of one or more Step 1 medications (typically generics) or Step 2 medications (Preferred Brands) before a Step 3 medication (Non- Preferred Brand) is eligible for coverage. When there are several equally effective medication choices available,
it makes sense to use the least expensive options first because it saves you and the plan money. The program covers 14 different classes of drugs used to treat some
of the most common chronic health issues and represent about 28% of all prescriptions filled. The goal of our step therapy program is to try lower cost alternatives, while avoiding interruption in therapy. In most cases, those that try the lower cost alternative find it works just as well and make the switch— helping lower their out-of-pocket costs and overall pharmacy spend. When you fill a step therapy prescription for the first time, UnitedHealthcare will send you and your doctor a letter advising that the medication is subject to step therapy requirements, provide information on the lower-cost, therapeutically equivalent alternatives, and instruct your doctor on how to request a prior authorization for the step therapy drug if its use is medically necessary and other alternatives cannot be used. After the first fill, you are responsible for the entire cost of the drug
if the step therapy program requirements are not met. For questions, call 1-800-444-6222.
Women’s Preventive Health Coverage
As required by the Affordable Care Act and in general, additional coverage for the women’s preventive health services such as gestational diabetes screenings, breast- feeding support, supplies and counseling and FDA approved contraception methods and contraceptive counseling
will be covered at 100% when care is received from
an in-network provider. And, at network pharmacies, certain generic and single source brand medications

How do I know what prescription medications can be filled through UnitedHealthcare Home Delivery Pharmacy?
In general, prescription maintenance medications are those taken on a regular basis to treat an ongoing health condition, such as asthma, diabetes, heart conditions and more. If your doctor has prescribed a medication for you to take every day, or several times a week, then you may
be taking a maintenance medication. To find out, you can ask your doctor directly; call United Home Delivery Pharmacy at the number on the back of your ID card or login to myuhc.com.

OptumRx, CVS and Walgreens make it easy for you to get your medications with options that may save you money. The Optum Rx Preferred 90 program allows you to get 3-month supplies of your medication(s) at CVS, Walgreens, or through OptumRx® home delivery – the choice is yours. For you, this means more cost savings, convenience and choice. To get started, simply bring your prescription into your nearest CVS or Walgreens location, or contact them online or via phone.
