Dragados 2022 Benefits Guide
Eligibility for Coverage
Employees
If you are a full-time domestic employee of Dragados working 30 hours or more per week, you are eligible to enroll for coverage on the first of the month following date of hire.
Note: The Dragados Health and Welfare Benefit Plan includes coverage for other employees and provides for other benefits that are not described in this booklet.
Dependents
If you enroll for coverage, you may enroll your eligible dependents. Eligible dependents include:
• Your lawful spouse— the term spouse also includes your same-sex partner in jurisdictions that legally recognize same-sex marriages
• Your domestic partner (same or opposite sex)—You must complete the required Affidavit and submit to HR for approval (see Human Resources for more information, other tax implications may also apply).
• Your child until the end of the month in which your child attains age 26. Eligible dependent children include:
» A child born to you.
» A child legally adopted by you from the start
of any waiting period prior to the finalization of the child’s adoption. It also includes a newborn infant who is adopted by you from the moment you take physical custody of the child upon the child’s release from the hospital prior to the finalization of the child’s adoption.
» A stepchild.
» A foster child.
» A child for whom you are the legal guardian. » For Voluntary Life Coverage, the following
eligibility rules apply:
» A child who is 19 or more years old but less than 26 years old, enrolled in a school as a full-time student and primarily supported by you;
» A child who is 19 or more years old, primarily supported by you and incapable of self- sustaining employment by reason of mental or physical handicap.
• If your Domestic Partner has a child, that child will also be included as a dependent. Benefits for a dependent child will continue until the last day of the calendar month in which the limiting age is reached.
• Your child who is age 26 or older (19 or older for voluntary life insurance, unless enrolled in school full-time), unmarried, and primarily supported by you and incapable of self-sustaining employment by reason of mental or physical disability which arose while the child was covered as a dependent under this Plan, or while covered as a dependent under a prior plan with no break in coverage. Proof of the child’s condition and dependence must be submitted to UnitedHealthcare within 31 days after the date the child ceases to qualify above. From time to time, but not more frequently than once a year, UnitedHealthcare may require proof of the continuation of such condition and dependence.
Important Notes
• Anyone who is eligible as an employee will not be considered as a dependent.
• No one may be considered as a dependent of more than one employee.
Enrolling for Coverage for New Hires
Most employees will enroll in benefits online. Follow the step-by-step instructions provided in your new hire materials. If you have any questions, please contact Human Resources.
You have 31 days from your initial eligibility date to enroll for coverage. If you are a new hire your initial eligibility date is the 1st of the month following date of hire. If you do not enroll for medical, dental, vision and/or the health/ dependent care flexible spending accounts within 31 days
of your initial eligibility date, you will default to “no coverage.” You will not be able to enroll until the next annual enrollment unless a qualified life event and/or special enrollment applies.
If you do not apply for supplemental life coverage within
31 days of your initial eligibility date, you and any eligible dependents will be considered late applicants. If you elect coverage at a later date, evidence of insurability satisfactory to Reliance Standard will be required.
