Creating true home
All too often, elders are asked to give up home just when they need it the most. In the Household Model,TM long-term care homes adopt a person-centered approach that shapes the physical environment, or- ganizational structure, and interpersonal relationships in ways that create an atmo- sphere of genuine home, while providing elders with clear opportunities to direct their own lives.
The Household ModelTM breaks down a traditional facility or new development
into households of 14-20 residents, with their own kitchen, dining room, living room and often the patios, porches and small cozy spaces you’d find in any home.
Each household has decision-making au- tonomy and is consistently staffed. Resi- dents get up when they want, bathe how and when they want, go to bed when they want, eat when and what they want and decide how they will spend their day. Household life is “normal,” spontaneous and full of new experiences.
From the first spark that tells you there should be something better, to the embers glowing in the fireplace after a pleasurable day at home, Action Pact is your guide, your support and your partner.
The Household Model TM creates true home for frail elders where they enjoy daily pleasures, direct their own lives and express their well-being
