Documentation to be communicated to receiving health care entity (implemented in Phase 2):
• contact information of practitioner,
• resident representative contact information,
• advance directive information,
• special instructions/precautions for ongoing care,
• the resident’s comprehensive care plan goals,
• all other necessary information, including a copy of the residents discharge summary.

• Resident assessment instrument. A facility must make a comprehensive assessment of a resident's needs, strengths, goals, life history and preferences, using the resident assessment instrument (RAI).
