investigation report for the off job fatal MVA:
CRISOD conducted a thorough investigation for the two off-job fatal accidents occured at CRISOD. The reports were presented to all CRISOD employees and shared with ISO management.
incident lesson learned:
CRISOD developed a process to share safety messages and incident’s lesson learned with all employees on a frequent basis. Sixty five (65) messages were shared through this process.
