Tumble Forms 2® Tadpole® Pediatric Positioner. Designed specifically for infants from birth to age three with cerebral palsy, motor delays or other motor dysfunctions. They benefit from this multi-use unit when used as an essential part of an effective therapy program. The latex-free Tadpole positioner includes a car- rying bag to conveniently store all modules: base (19”W x 34”L), log roll (8”W x 29”L x 4”H), half roll (4”W x 18”L x 2”H), wedge (17”W x 18”L) and tray with mirror (12”W x 8”D x 7”H).
• Prone: With the half roll and two lateral supports, the Tadpole positioner facilitates active body extension, head flexion, chest elevation and forward propping of arms for midline hand touching. Mirrored unit encourages childs curiosity. Place the log roll under the base for vestibular activities.
• Seated: Use base and wedge modules together for long leg or log roll sitting. Either side of wedge may be used depending on desired degree of upright or reclined position. Attached tray pro- vides additional support and a surface for eye-hand activities.
• Side-lying: Attach log roll to base for side-lying to facilitate control of lateral head and trunk movements. Prop base to posi- tion child for improved respiration. No color choice.

Tumble Forms® Maintenance Kit. In the event the Tumble Forms coating shows wear from heavy use over an extended period of time, a maintenance kit is available for making minor repairs. Any type of foam compatible adhesive can be used to repair the upholstery. The compo- nents may be used on any Tumble Forms item. Latex free. Maintenance Kit includes:
• Vinyl repair patches in assorted Tumble Forms colors
• Replacement Velcro® self adhesive hooks and loops: two 2” x 6” (5 x 15 cm) hooks, two 2” x 12” (5 x 30.5 cm) loop, and one 6” x 12”
