A change from union to non-union or vice versa
Residence Change. The employee or eligible dependent changes residence.
Significant Change in Cost of Coverage. The cost of elected medical, dental, or vision coverage significantly increases or decreases. Please note: An employment status change from full- to part-time, or vice versa, does not qualify as a significant change in cost of coverage.
Change in Dependent Care. The employee experiences a significant change in dependent care (i.e. change in providers) or in the cost of employment-related dependent care expenses.
Medicare or Medicaid Eligibility Change. The employee or eligible dependent gains or loses entitlement to Medicare or Medicaid.
Qualified Medical Child Support Order. The employee, spouse, or ex-spouse is required by a Qualified Medical Child Support Order to provide medical and/or dental coverage for a child.
The impact of the various status changes and the ability to change enrollment or coverage is dependent upon the status change. Employees should consult Human Resources for more specific information.
