Incidents & Injuries
All incidents and injury must be reporting using the Accident / Incident Form. If you are
the primary witness to this injury or incident with anyone on campus, it is your
responsibility to file the report. A blank report is located in the back of your guide for
reference. When the incident takes place, take necessary action. After the incident is
tended to, fill out a report at the front desk and turn it in for filing. Please fill this form out
completely and carefully because it will serve as legal documentation should the incident
ever go to litigation.
