Community Agency Referral Process – Pathway to Positive Mental Health
Step 1
A possible mental health concern is recognized. This concern may be identified by a teacher, parent , friend, or self.
Step 2
Fill out the Mental Health Support Request form. This request can be found at a location designated by the school. This completed form is submitted to the Certified School Counselor or assigned School Social Worker.
Step 4
Refer to the Individual Problem Solving Team (IPST) if these intervention efforts show limited results and the student's school functioning is impacted.
Step 3
The Certified School Counselor or assigned School Social Worker will review the mental heath request and consult (with referer) in regards to needed supports. Classroom supports, outside classroom intervention, and/or make approprriate community referrals will be discussed.
Communicate with parents. Document intervention plan.
