NACDD hosts a database of more than 400 public health success stories from state and territorial departments working in chronic disease prevention and control.These case studies are shared with legislators, partners, and national advocates. Submissions are welcome through a simple form at publichealthsuccess. org.

NACDD’s annual Impact Awards provide those who work in the eld of chronic disease with an opportunity to be recognized for best practices in chronic disease prevention and health promotion, and other contributions that go beyond public
health. Nominations are solicited each summer, and NACDD’s Awards Committee, made up of Board Members, selects the winners each fall. For more information on the Impact Awards, contact the Member Services department at Members@ chronicdisease.org.
