After Hospital Care
Before you are discharged, make sure you understand what comes next. If you need other care options after you leave—discuss this with your healthcare team and be sure your support person is with you when you ask. Options after your hospital stay may include:
Home Health.  is covers a wide range of healthcare services that can be given in the home for an illness or injury:
• Personal care: including bathing, dressing and eating
• Healthcare: physical, speech
& occupational therapy or skilled nursing
Independent Living.  is usually refers to apartments or communities designed for seniors over 55 who do not need medical care or nursing sta . Amenities are o ered, such as social activities,  tness, meals, and housekeeping.
Assisted Living.  ese communities
are designed to provide assistance with basic activities of daily living such as bathing, grooming, and dressing. Some states also allow assisted living to o er medication assistance and/or reminders.  ese communities o er a more home-like atmosphere.
and require specialized medical care with extended recovery times.  e care needed in an LTAC o en includes pulmonary, wound, transitional or sub-acute care, dialysis, IV antibiotic therapy, and pain management.
Nursing Home. A nursing home provides personal and medical care for those patients who cannot be cared for at home. Most nursing homes have nursing aides and skilled nurses on hand 24 hours a day.  e sta  provides medical care, as well as physical, speech and occupational therapy.
Hospice.  is care plan is for people whose life expectancy is six months or less, and involves palliative care (pain and symptom relief) rather than ongoing curative measures. Hospice typically provides services, such as speech and physical therapy, therapeutic massage, dietary assistance, bathing and other personal care services, emotional, and spiritual support and grief counseling for family members.
Rehabilitation Hospital
Rehabilitation Hospitals o er medical rehabilitation for patients who have su ered traumatic injuries or illnesses. Their goal is to restore some or all of the patient’s physical, sensory and mental capabilities that were lost due to injury, illness or disease. Speech, occupational and physical therapies are provided for patients who are medically stable and can tolerate at least 3 hours of therapy a day.
LTAC. LTAC stands for Long Term Acute Care. It is a specialty-care hospital for patients that are seriously ill or injured

Billing & Financial Information
Hospital Bill
You will receive an itemized bill for hospital services from CHI St. Luke’s Health– e Vintage Hospital. Please review the bill before you pay it. If you have questions regarding your bill, contact a patient  nancial services account representative at 832-534-5110.  is bill does NOT include professional services provided by your physician.
Physician Billing
Your physician, other physicians who conducted tests or procedures and/
or consulting physicians chosen by
your primary physician will bill you directly for their professional services. Depending on your insurance plan, you may be responsible for paying these bills in addition to the hospital bill. If you have questions about a physician’s professional fees, please contact the physician’s o ce listed on the billing statement.
Insurance
If you give permission, the hospital will  le a claim with your insurance company, including Medicare, a er your discharge.  e insurance company may determine that you are responsible for some or the entire claim. CHI St. Luke’s Health– e Vintage Hospital will bill you for any amounts not covered by your insurance.
