  WHAT TO DO FIRST – A CHECKLIST
 Notify these people as soon as possible:
The funeral home to arrange for the transfer of the deceased into our care and to set an arrangement time in order to reserve your time requested for funeral services
Relatives, executor/executrix and friends
Employers of deceased and ask if there is any death benefit
Pension companies
Insurance agents (life, health and accident)
Current and urgent bills (i.e. mortgage, rent, taxes, telephone etc.)
Religious, fraternal, civic, veterans organizations and unions, if applicable
Lawyer and accountant
Doctors/Dentist in order to stop appointment notices
Statistics Needed:
Education Level
Deceased Hebrew name and father’s first name
Full legal name – other names must be identified by “also known as” (AKA)
Legal mailing address
Name of business or employer’s name, address and type of business
Industry and occupation
Military service honorable discharge (DD-214)
Date and place of birth
Social security number
Citizenship
Father’s name and birthplace
Mother’s name, maiden name and birthplace
Locate the will
During the arrangement or pre planning conference you will discuss:
Clothing or shroud that the deceased will be buried in
Where your loved one is to be interred
The Funeral Director will go over cemetery options; for example are we using a family owned plot?
Service details – would you like a chapel or graveside ceremony?
Whether a Rabbi, a family member or both will be conducting the service?
Casket selection
Are there any special requests that need to be addressed, conveyed or announced?
