Form PU-101 (2017-18 Club Officer Reporting)
Required information of the Club and the New Officers (2017-18):
1. President, 1st/2nd Vice Presidents, Secretary, Treasurer, Membership Chair, LCIF Coordinator and Directors. Local titles for the Club can be created and added to the Officers.
2. Officers' names and Member number (member number/full name are found on the semi-annual membership dues invoice from LCI).
3. Updated home address, phone number, email address

7. Club meeting day(s) of the week & meeting time (hour)

9. Club mailing address (Lions mail will be sent to home address unless "send mail to club address" is
provided and checked.
10. Officers' choice of receiving their mail (To select: Club address or Home address)

To download the form, visit www.lionsclubs.org, search for PU101, double-click "PU101 EN". (The Form appears can be filled online and saved for records/printing)
Print both the pages and complete it in all respect.
Mail it to LCI with a copy to DG and 1st VDG.

To submit the report through MyLCI:
Logon to http://mylci.lionsclubs.org
From drop-down arrow of "My Lions Club" (in blue banner), select "Officers" to get current Officers page.
From drop-down arrow of "Select Term", click on "next year" which takes you to a new page of "Officers"

To fill in the position of Club President, click "Add Officer" button against that title which takes you to a new page; click "select Member" button when the list of club members' full name and current position (if any) appears. Click on that correct name, and then click on the "save" button.
Repeat this process for other positions-Club First Vice President, Club Second Vice President, Club Secretary, Club Treasurer, Club Membership Chairperson and Club Directors.
Local titles for the Club may be created and saved to assign the positions to the concerned members.
Go back to "My Lions Club" drop-down arrow, select "members" which brings a page of members' information. Verify the information of each of those new officers and correct them as needed by clicking "Edit Member" button against each name. Input the correct information and "save".
After all the editing is completed, go back to "My Lions Club" >Officers >Select Term, then "next year" to bring out the page of all new officers; verify the correctness of these officers' information.
To view the newly completed PU101 Form, from drop-down arrow of My Lions Club, select 'Reports', then click on "Lions Club Officer Report for Next Year" and click "view/print" button in right-side to view, save or print the page of New Officers' list along with club's meeting place/days/time etc.
