The most important is the Hippa compliant parental consent form and a template is available that can be customized with your clubs’ information. You must have a signed consent form before you screen a child. These forms can be given to the daycare center for distribution prior to the screening day. There is also a form that the school should fill out with the child’s information that can be pre-loaded into the Spot device before the day of the event.
Screening results in most cases can be downloaded to the school administration so that they can disburse this information to the parent; however, each device has a wi-fi capable printer available if you choose to print the results out for the parent. This would most likely occur at a community event such as a health fair.
Apart from the Dallas Filipino Lions Club, we are all fairly new to this and learning as we go, but there is enough information, training and support that I feel certain that each club in District 2-X1 can have a successful Lions Kidsight USA screening event. If you need more information, contact me. Our goal is to have 20 clubs hold their first vision screening by the end of this Lions year.... will your club be one of those?

OUR CHILDREN Are Counting on Us Waxahachie Lions Club Receives Award
Waxahachie Lions Club has received the 2017 Waxahachie ISD’s Partners in Education Award. This special award is given to businesses who have gone above and beyond to benefit the entire local district in relation to the size of their business and organization. The PIA board is not required to bestow this award every year and no business or organization can receive the award more than once in a five-year period.
The Lions Club of Waxahachie partners with WISD in innumerable ways.
This year they updated backstops on multiple campuses within WISD providing updated playing fields for WISD students. They support the Adopt-a-Class program adopting two classrooms, on separate campuses with multiple volunteers involved in both campuses. Waxahachie Lions Club also participated in the annual DEAR (Drop Everything and Read) Day program providing reading role models. They provide eye exams for students in need of this service in grades K-12 throughout the district. They support WISD seniors by providing scholarships for deserving students. A big THANKYOUtotheLionsoftheWaxahachieLionsClub for your outstanding partnership with Waxahachie Schools demonstrating what it means “WE SERVE”.
