In some cases, medical management of otitis externa may not be appropriate and surgery such as a total ear canal ablation and bulla osteotomy should be considered. Cases where this may be appropriate include where there are severe secondary changes to the ear (Figure 6), if medical treatment fails or is not possible due to patient or client factors or when otitis media is present with structural changes to the bulla.
